The Coach Sarna League - Medical Release Waiver:

I, the parent/legal guardian of the above-named player, a minor, agree that both the player and I will abide by the rules and regulations of The Coach Sarna League.

For myself, the player, and our respective heirs, administrators, and successors, intending to be legally bound, I hereby release and indemnify The Coach Sarna League, the owners and operators of the facilities used for the programs, and their respective directors, officers, employees, agents, and representatives from any and all claims, liabilities, damages, or causes of action arising out of or in connection with the player’s participation in a The Coach Sarna League program activity. This includes, without limitation, the player’s transportation to/from any activity, which is hereby authorized.

As the parent/legal guardian of the above-named player, I hereby consent to emergency medical care as prescribed by a duly licensed Doctor of Medicine or Doctor of Dentistry. This care may be administered under whatever conditions are necessary to preserve the life, limb, or well-being of myself or my dependent.
